Download the "Guest Statement" (Excel File) from the following, fill it out, and send it by email or fax.*Please submit at least 1 month before the training camp.

Reservation is complete. Please be careful on the day.- Cancellation of reservation
In accordance with the bylaws of the Gifu Prefectural Sports Science Center.
・No cancellation fee will be charged for cancellations made up to 7 days prior to the date of use of the facility.
・In case of cancellation from 6 days to 2 days prior to the date of use, the cancellation fee will be half of the usage fee.
・Cancellations made on the day of use will be charged the full amount of the fee.
※Excluding natural disasters and other events not attributable to the user - Schedule changes
Please inform us of the group's schedule change at least 3 months in advance. Please refrain from making changes after that. - Number change
Please inform the facility whenever changes occur. Please contact us at least 1 month in advance. The last five or more changes cannot be handled. - Lack of food
The facility includes 3 meals per night. If you are missing a meal, please let us know before the training camp. It is not possible to respond to sudden missing meals that occurred during the training camp. Please note. - Guest Statement
